Middle School Admissions Forms: 2012-2013 School Year

6th Grade Application Materials

The ISB Middle School application must include the following:

  • Middle School Application form (including Parent Statement) and $75 Application Fee
  • A copy of student’s most recent school report card
  • Student’s ISEE (Independent School Entrance Exam, see www.erblearn.org / ISB’s #: 336654)
  • Teacher Evaluation Form #1
  • Teacher Evaluation Form #2
  • Applicant Statements (see Application for instructions)

Click on the links below to download the files. They may be filled out electronically or by hand and mailed, emailed or faxed to ISB. Note that applications are not considered until the $75 non-refundable application fee has also been submitted.
